Assessment

  • Urinary and bowel patterns: Document voiding frequency, amount, and appearance of urine. Note any history of incontinence, constipation, or diarrhea.
  • Fluid intake and output: Monitor intake to ensure adequate hydration and compare it to output to identify potential issues.
  • Risk factors: Assess for potential causes such as medications, underlying medical conditions, or neurological involvement.
  • Skin integrity: Check for skin breakdown, especially in areas prone to moisture from incontinence. 

Nursing interventions

  • Bladder and bowel retraining:
    • Timed voiding: Schedule toileting on a regular schedule (e.g., every 2 hours) to gradually increase time between voiding and bowel movements.
    • Scheduled toileting: Help the client establish a regular time for bowel movements, such as after a meal or with a scheduled laxative.
  • Promoting fluid intake: Encourage adequate daily fluid intake to support normal elimination, with a goal of 2,000 to 3,000 mL per day, unless contraindicated.
  • Dietary adjustments:
    • Increase fiber intake through fruits, vegetables, and whole grains to promote bowel regularity.
    • For diarrhea, suggest a BRAT diet (bananas, rice, applesauce, toast) and avoid irritating foods.
  • Assistive devices: Insert and manage urinary catheters or perform irrigations as needed.
  • Medication management: Administer prescribed medications, such as stool softeners or alpha-blockers, to aid elimination.
  • Comfort and skin care:
    • Provide a comfortable and private environment for toileting.
    • Use protective ointments like zinc oxide for skin protection if needed.
    • Clean the periurethral area with water before catheterization, as this is as effective as antiseptic cleaning. 

Client and family education

  • Explain the importance of establishing a toileting schedule.
  • Provide instructions on managing symptoms and preventing complications.
  • Discuss the role of diet and fluid intake in elimination. 

Psychosocial support

  • Use therapeutic communication to address the embarrassment associated with incontinence.
  • Provide emotional support to help manage feelings of shame, guilt, or low self-esteem related to the disorder.
  • Involve family members or caregivers in the care plan to ensure consistent support.
